Salary: £25,364 + extra payments for on call.

Driving licence is required.

Working Hours: Monday to Friday 9am - 5pm with additional on call duties on a rota basis.

Job Description:

The Care Coordinator plays a key role in ensuring the highest quality care services by managing care workers and performing field‑based duties. Responsibilities include assessing and reviewing individual care plans, as well as supervising, instructing, and supporting care staff in the field. This role requires a valid driver’s licence and access to your own vehicle due to the travel involved.

You are an efficient, organised Care Coordinator passionate about community care. You excel in managing rotas to impeccable standards and pride yourself on building strong relationships with care workers.

Stepping Up? If you have previous healthcare experience and a solid understanding of CQC, care plans, and rotas, we’re here to support your transition into this role. NVQ level 3 is desirable.
