Job summary

DarlingtonCommunity Services are looking to recruit a Clinical Administration Assistantto support the team. This post will be based in Hundens Lane, Darlington,taking referrals for the Community Nursing Team. The post holder would berequired to work with and support the team with their clinical administrativefunctions associated with the management of patients: accepting incoming tasksand referrals to the team and setting appropriate care plans. Using theautoplanner functionality within SystmOne to allocate the following daysvisits. Support the duty Sister to review allocated visits and assisit with anychanges to the visit lists.

We provide hospital services from two acute sites – Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients\’ homes.

Supporting the duty sister/seniornurse accepting electronic referrals and tasks, action appropriately andapplying applicable care plans; follows SOPs, procedures and treatment planswhilst seeking advice if required.

Managesown workload with an understanding of what needs to be done, when and why inorder to assist the function of the team.Has the ability to flex and prioritise if required due to an unexpectedor urgent need.

Providestaff with advice re administrative procedures and help with access asrequired. This includes general adminduties around RA agent and IAA functions.

Coordinatingclinical work for colleagues, using the autoplanner functionality withinSystmOne. Work will be allocated for thefollowing day and checked for nuances and discrepancies with the dutysister/senior nurse. This will include a review of allocated visits to ensurethey are as geographically close together as practical while maintainingcontinuity of care to our patients.

Triageincoming requests from health and social care professionals and patientsdirectly supporting the duty sister/senior nurse for the shift.

Communicatewith patients, relatives and health care practitioners effectively who may bedistressed and/or challenging at times.

Demonstratingeffective time management skills and participates in the monitoring andreviewing of team performance.

Gatherkey audit data from electronic systems and produce reports to Team Lead asrequested and within time scales.

Promotean environment where health promotion and informed patient choice is encouragedand developed to its maximum potential, within the available resources

Toundertake relevant training to enable the provision of a high quality service.

Tocomplete effective record keeping in accordance with Trust policy and legalrequirements.
