This is an exciting opportunity to work as part of the multi-disciplinary Diabetes Service at the Countess of Chester NHS trust.

The Lead Diabetes Specialist Nurse (DSN) for Insulin Pump and Hybrid Closed-Loop Systems will provide specialist clinical leadership, expertise and support in the delivery and development of advanced diabetes technologies across the diabetes service.

The post holder will have responsibility for leading and developing the insulin pump and hybrid closed-loop service, ensuring that patients with Type 1 diabetes receive safe, evidence-based, person-centred care. The role will involve supporting patients commencing insulin pump therapy, optimising existing therapy, and providing ongoing education and clinical support to patients using continuous glucose monitoring (CGM), insulin pumps and hybrid closed-loop systems.

Working as an integral member of the multidisciplinary diabetes team, the post holder will contribute to the strategic development of the diabetes technology service and ensure that it remains responsive to the changing needs of patients and advances in diabetes technology.

Main duties of the job

Provide specialist nursing leadership and clinical expertise in insulin pump therapy and hybrid closed-loop systems. Provide specialist advice during periods of illness. Lead the development and expansion of the insulin pump and hybrid closed-loop service. Work collaboratively with the wider multidisciplinary team to ensure consistent and integrated care. Provide specialist education to patients and families regarding insulin pump therapy, CGM and hybrid closed-loop systems. Ensure practice is compliant with current national guidelines, professional standards, Trust policies and local diabetes pathways. Maintain a high level of clinical competence and professional accountability. Participate in clinical audit, research, service evaluation and quality‑improvement activity. Provide specialist advice and education to members of the multidisciplinary team regarding diabetes technology. Contribute to multidisciplinary case discussions and complex patient reviews. Communicate complex clinical and technical information clearly to patients, relatives and healthcare professionals. Manage sensitive and difficult conversations appropriately. Advocate for patients and ensure that their preferences and individual circumstances are considered in clinical decision-making. Demonstrate a high level of IT competence and confidence in using diabetes technology platforms.

The Trust’s services are provided from three locations:

  • The Countess of Chester Hospital: providing 473 general and acute beds.
  • Ellesmere Port Hospital: providing 60 beds as a rehabilitation, intermediate and outpatient facility.
  • Tarporley War Memorial Hospital: a base for community services which serve the local rural population.

Qualifications

  • Registered Nurse, RGN/RN (Adult).
  • Diploma/degree in nursing or equivalent experience
  • Nurse Prescriber or working towards this qualification.
  • MSc in relevant topic.
  • Teaching qualification.

Knowledge

  • Teaching experience.
  • Post registration Diabetes Qualification.
  • Experience in service development and implementation of change

Skills

  • Able to influence and overcome resistance through application of advanced communication skills
  • Able to present information to professional groups.
  • Able to supervise and support junior members of the team
