The Countess of Chester Hospital NHS Foundation Trust is seeking a permanent Advanced Clinical Practitioner to work within the Same Day Emergency Care (SDEC) and Emergency Department footprint at the Countess of Chester Hospital.
The ACP will work with the multidisciplinary team and autonomously to provide clinical care, expertise and advice to support same day discharge and prevent unnecessary admissions. A background in Frailty would be advantageous.
