Overview
Specialist Physiotherapist role at Countess of Chester Hospital, based in Ellesmere Port (Custom House). The role delivers Urgent Community Response (UCR) and Virtual Ward services within the Hospital at Home Service. The service operates 7 days a week from 08:00–20:00.
Responsibilities
- Provide a 2-hour response to patients experiencing a health crisis to support treatment at home and avoid hospital admission.
- Provide therapy assessment and treatment for patients on the virtual wards to optimise physical function.
- Perform specialist therapeutic assessments autonomously, use clinical reasoning to diagnose and deliver and adapt individualised treatment programmes within the patient’s home.
- Work in collaboration with Hospital at Home Team, Community Respiratory Team, Community Care Teams, Northwest Ambulance Service, Social Services, Millbrook and the patient’s registered GP to support the patient at home.
- Allocate appropriate caseloads for Therapy Assistants and work flexibly in response to service needs.
- Contribute as a member of a holistic team including GPs, Advanced Clinical Practitioners, Registered Nurses, Therapists, Therapy Assistants, Pharmacists and Clerical Support.
Location and Hours
The role is based at Custom House, Ellesmere Port, within the Hospital at Home Service. It is a 7-day service operating from 08:00–20:00, and successful applicants will work shifts across 7 days.
