Working Part time: 24 hours per week
An exciting opportunity has arisen to work within the Rapid Response team covering Cheshire West and Chester. Due to innovative expansion and integration there will also be a new base at Tarporley Community Hospital.
Role Summary
All of these roles involve working with Community Support Workers, Physiotherapists, Occupational Therapists and the wider multidisciplinary team. The main objectives of the team are to expedite and facilitate safe and timely discharges from wards and emergency department areas, and provide resettlement, rehabilitation, care and medication support at home. The post holder will need to maintain a strong patient‑centred focus, whilst being adaptable, autonomous, relate well to colleagues across a range of disciplines in hospital and community, and be able to work under pressure.
Key Responsibilities
You will be responsible for identifying and assessing patients located on wards and in the emergency department for suitability at home with Rapid Response Team intervention. You will be expected to expedite discharges and provide assessment, nursing care, advice and information to patients, relatives and carers within the home or appropriate care setting. You will be aiding the Community Support Workers and Therapy staff to maximise patients’ independence and optimise their safety and quality of life, to maintain patients’ in their home, where possible.
