The National Health Service (NHS) has a number of sub‑acute and community schemes that have developed in recent years from guidance of NHS white papers such as the NHS Long Term Plan (2019) and the NHS Five Year Forward View (2014), which recommend that NHS trusts deliver more care and services closer to a person’s home rather than in acute hospitals.

As pressure on Emergency Departments and hospitals across the UK grows, new care models will be pivotal in delivering and relieving some of these pressures by diverting work away from the hospital and into the community. The NHS England 2021 paper “Community health services two‑hour crisis response guidance” aims to address this pressure.

In Cheshire West the Hospital at Home team continues to expand its offer of crisis same‑day response and virtual wards. The workforce fits the NHS model well in supporting hospital‑level care at home; experienced general practitioners, advanced clinical practitioners with prescribing rights, registered therapists and registered nurses offer a 7‑day service allowing for wrap‑around care and an excellent patient experience as evidenced in the service’s monthly feedback from patients.

Role Summary

We are looking for a hard‑working, enthusiastic, highly motivated and experienced clinical lead therapist to join our NHS@Home directorate to support the Hospital at Home team in admission avoidance and the care of patients on virtual wards.

The primary focus of the role is to work as an advanced autonomous practitioner providing patient‑centred clinical care, encompassing the skills of assessment, treatment and discharge of patients. The post is intended to provide improved outcomes for our patients by providing safe, kind and effective care.

Responsibilities

Provide advanced, autonomous clinical care to patients in the community, including assessment, treatment and discharge planning. Support admission avoidance for hospital and care of patients on virtual wards. Work in a 7‑day service, collaborating with GP, clinical practitioner and nursing teams.

Qualifications and Experience

Experience managing complexity within the home environment and providing care for the ageing population. A background in therapeutic practice, with strong assessment, treatment and discharge skills is desirable.

  • The Countess of Chester Hospital – 473 general and acute beds.
  • Ellesmere Port Hospital – 60 beds as a rehabilitation, intermediate and outpatient facility.
  • Tarporley War Memorial Hospital – base for community services serving the local rural population.
