Working Full time: 37.5 hours per week. An exciting opportunity has arisen at the Countess of Chester Hospital for a permanent Advanced Clinical Practitioner (ACP) to work within the Same Day Emergency Care (SDEC) and Emergency Department (ED) footprint at the Countess of Chester Hospital.

Key Responsibilities

  • The successful ACP will work collaboratively with the multi‑disciplinary team but also work autonomously to provide clinical care, expertise and advice supporting same day discharges and preventing unnecessary admissions.
  • A clinical background in Frailty would be advantageous.
  • The Advanced Clinical Practitioner role works beyond normal professional boundaries as an Allied Health Professional (AHP) and will be working at an advanced level, undertaking tasks that have previously been undertaken by medical and/or technical staff.
  • The Practitioner will practice autonomously and also within a defined team / multi‑disciplinary team to provide advice and support to patients, carers, and healthcare professionals, in accordance with local guidelines and policies.

  • Ellesmere Port Hospital: providing 60 beds as a rehabilitation, intermediate and outpatient facility.
  • Tarporley War Memorial Hospital: a base for community services which serve the local rural population.
