As a Health Coach you will support individuals on their journey to type 2 diabetes remission using our ground breaking programme. This will involve regular coaching sessions, in person, in the Bury St Edmunds area. You will be supported by our expert team with remote training and ongoing mentoring and development. You will be part of a wider team of local and remote health coaches supporting our NHS commissioned 'Path To Remission' service.
Responsibilities:
- Providing evidence-based nutrition and health behaviour change support to assist individuals to make changes to their eating and activity behaviours.
- Facilitating all sessions using motivational interviewing skills to guide and support individuals to make long term behaviour changes.
- Offering a personalised, person-centred service to individuals and adapting the programme delivery to meet their individual needs, e.g. cultural, medical, psychological, disability etc.
- Maintaining accurate and up-to-date clinical records using our secure electronic medical record system, ensuring the proper management of sensitive data in line with company policy.
- Engaging in ongoing training relevant to your role and evidencing professional development.
- Working with your Counterweight colleagues to contribute to service developments – we’re always looking to share knowledge and resources.
- Ensuring due consideration and adherence to our internal Counterweight policies and procedures for reporting clinical risk incidents, adverse events.
- Following Counterweight medical monitoring guidelines and acting on thresholds of actions for measurement reporting according to established protocols.
- Sharing our company values and driving our company mission across our services.
Qualifications:
- (one of the following): Associate or Registered Nutritionist (AfN), Registered Dietitian (HCPC), Qualified Health Coach, Trainee Health or Clinical Psychologist (BPS approved programme)
Essential: 1+ year experience in health coach role
Desirable: Experience working with people living with obesity and/or type 2 diabetes
