COTA Case Management is a dedicated team of Rehabilitation Case Managers who specialise in providing immediate needs assessments and case management services for individuals who have suffered injuries. The team focuses primarily on supporting those impacted by complex orthopaedic trauma and amputation. With expertise and compassion, they help clients navigate their rehabilitation journeys, ensuring tailored services that promote recovery and independence. COTA’s approach is centered on delivering effective outcomes and high-quality support to clients in need.

We are looking for Associate Case Managers based in the north west and midlands. The Case Manager will conduct immediate needs assessments and manage individualised rehabilitation plans for injured individuals, ensuring their recovery and rehabilitation goals are met. Responsibilities include:

  • Liaising with healthcare professionals
  • Coordinating treatment and recovery services
  • Providing guidance to clients and their families throughout the rehabilitation process
  • Documentation and monitoring progress
  • Maintaining effective communication with stakeholders

Qualifications:

  • HCPC/NMC registered health professional, with a minimum 3 years rehabilitation case management experience, including the management of amputee clients
  • Member of CMSUK or BABICM. Registered with IRCM (or working towards)
  • Strong communication, interpersonal, and problem-solving skills
  • Ability to comprehensively assess immediate needs and develop tailored rehabilitation plans for clients
  • Exceptional organisational skills with attention to detail in documentation and record-keeping
  • Understanding of limb loss, latest prosthetic advances, complex orthopaedic trauma, healthcare, and recovery processes
  • Proficiency in collaborating with healthcare professionals and stakeholders, including collaborative working with solicitors and insurers
  • Knowledge of relevant regulations and processes is desirable
  • Up to date CPD and proactive approach to personal development
