We are seeking a dedicated and compassionate Social Worker to join an All‑Age Disability (0-25) service. This role supports children, young people, and young adults with disabilities that significantly impact their daily lives. You will work closely with individuals and families to identify needs, develop outcome‑focused plans, and ensure a smooth transition through different stages of life. Key Responsibilities
- Work in partnership with children, young people, young adults, and their families to assess needs and develop tailored social care plans.

Essential Requirements
- Qualified Social Worker with Social Work England registration.
