Age UK Cornwall & Isles of Scilly are seeking an additional team member to join the Gateway Outreach team, who support adults through person-centred, strengths-based conversations focused on "What Matters" to the individual.
Using the My CAW (Measuring My Concerns and Wellbeing) assessment, this role will involve co-producing with the client, Personalised Support Plans that promote self-management of health and wellbeing, early intervention, and prevention.
