An exciting opportunity has arisen for a Trauma-Informed Social Prescriber to support the development and delivery of a Trauma-Informed Management (TIM) Programme within the St Austell Integrated Neighbourhood Team. This role combines the relationship-based, community-focused strengths of social prescribing with a wider leadership role in embedding trauma-informed practice across neighbourhood services and voluntary sector partnerships.
The post-holder will work alongside individuals experiencing complex social, emotional and practical challenges including trauma, adverse childhood experiences (ACEs), poverty, homelessness, isolation, domestic abuse, poor mental health, addiction, or repeated crisis presentations.
The role will focus on:
- Building trusted relationships with individuals and communities
- Supporting personalised care and support planning
- Improving coordination between health, voluntary and community services
- Reducing crisis-driven demand through proactive engagement
- Supporting people towards improved wellbeing, social inclusion, learning, volunteering and employment opportunities
- Embedding compassionate and psychologically informed approaches across teams and partnerships
The Trauma Informed Social Prescribing role will support people on their journey to wellness, social inclusion, learning or work. You will manage a caseload of members with high complex needs and help them to identify and overcome their personal barriers to achieving their goals focused on training, education, volunteering, and work. You will be responsible for supporting participants in taking positive action, and at the centre of this is the individual and what matters to them.
