You’ll provide home-based, patient-centred care across Callington and Launceston with a multidisciplinary team, delivering high-quality nursing care in patients’ own homes.
You will assess and plan care, promote independence and work with GPs, therapists and social care colleagues to support wellbeing. Flexibility for travel and varied shifts is required.
