The team is focused on delivering high standards of care, providing primary liaison role for primary care and community staff, working in primary care and community settings, co-coordinating primary care services to provide better health outcomes for people with a learning disabilities. You should possess commitment to team working and innovative delivery of excellent standards of care.

Ability to travel independently across the County to fulfil the requirements of the post is essential.

This is a full time post working 37.5 hours a week.

Main duties of the job

The purpose of this post is to provide primary liaison role for primary care and community staff, working in a primary care setting, co-ordinating primary care services to provide better health outcomes for people with learning disabilities.

A key part of the post involves ensuring the uptake of annual health checks for individuals with learning disabilities and supporting individuals to access health screening checks. The post requires close working with GP Surgeries and hospitals.
