Clinical Prioritiser Overview

North & East Integrated Transfer of Care Hub (ITOCH) Team are seeking to recruit a clinical prioritisers to coordinate the local demand and capacity, and to ensure that individuals needs are triaged, prioritised and addressed through the most appropriate resource from the multi-disciplinary and cross organisational team.

We encourage applications from Band 5 staff looking to develop themselves.

Responsibilities

  • Prioritisation – locality based decision-making enables effective prioritisation of care delivery across existing team boundaries.
  • Capacity – localising delivery and ensuring effective flexibility of workforce improves efficiency of service delivery.
  • Communication – enhanced communication speed up escalation and add flexibility to response.
  • Utilise RiO to triage and process referrals through the new STRATA system, coordinating demand and capacity to ensure the highest needs are prioritized and addressed with the most appropriate available resources from the multi-disciplinary and cross organisations team.
  • Provide specialised knowledge and exceptional skills in managing frailty and intermediate care, supporting advanced assessment and care planning to people suffering from a range of complex conditions, ensuring a seamless pathway through acute and community services.
  • Manage frequent travel to various locations throughout the county, demonstrating how to manage these travel requirements.

Qualifications & Expectations

Band 5 staff; experience in community-based care desired. The role requires frequent travel, which may not be accessible by public transportation. Successful candidates will need to demonstrate how they will manage these travel requirements.

We’re an NHS community and mental health provider Trust based in Cornwall and the Isles of Scilly, delivering community and hospital-based care to improve people’s physical and mental health. We also provide specialist support to people with dementia or a learning disability.
