We are seeking a Practice Nurse to join our small friendly surgery. The successful applicant must be well motivated and flexible as our Practice only employs one Practice Nurse and a Health Care Assistant. Confident and competent in reviewing patients is essential. A good knowledge of QOF - Chronic disease management is essential. The applicant will need to work autonomously.
Main duties of the job include:
- Childhood immunisations
- Long term condition reviews
- Cervical Cytology
Main duties will be to provide an efficient and well organised general nursing role, working closely with the GP's and other clinical staff.
About us: We are a small family practice based in Bolton. We are a friendly team and pride ourselves on providing high quality patient care. We have a list size of 4100 patients, supported by a team of GP's and ANP. We have full access to a range of ARRS staff including MSK and Pharmacists. The clinicians are supported by a Practice Manager, Assistant Practice Manager, 1 Secretary, 5 Receptionists and 1 Health Care Assistant. We use EMIS clinical system.
Job responsibilities include:
- Assess, plan, develop, implement and evaluate programmes which promote health and wellbeing, and prevent adverse effects on health and well being.
- Assess, plan, develop, implement and evaluate individual treatment plans for patients with known long term conditions.
- Proactively identify, diagnose and manage treatment plans for patients at risk of developing long term conditions.
- Work with other health care professionals to diagnose, monitor, manage and treat long term conditions.
- Perform various health checks and procedures including blood pressure, pulse rate, temperature, venepuncture, glucose tolerance test, and vaccinations.
- Support patients to adopt health promotion strategies that encourage healthy living.
- Deliver opportunistic health promotion using opportunities such as new patient medicals.
- Provide information and advice on medications and manage the needs of women attending for cervical cytology.
- Implement and participate in immunisation programmes for both adults and children.
- Oversee clinical stock systems to ensure safe and sufficient supplies.
- Take lead responsibility for vaccine management and cold chain compliance.
- Maintain general tidiness and cleanliness of nurses and clinical rooms.
- Recognise and work within own competence and professional code of conduct as regulated by the Nursing and Midwifery Council (NMC).
- Participate in the maintenance of quality governance systems and processes across the organisation.
- Work effectively as part of a team, supporting others and exploring new ways of working.
