Cornerstone Practice and Healthcare is seeking a Care Coordinator to manage the Local Enhanced Services (LES), coordinate Multidisciplinary Team (MDT) meetings and End of Life Care support across four sites in Blackburn and Oswaldtwistle.
You will work with GPs, nursing staff and the Quality Manager to ensure patient cohorts are identified, reviewed and supported, delivering high‑quality care with a warm, patient‑centred approach.
