About the Department: At Corewell Health, the Care Management teams consist of over 1,000 professionals, including Nurse Care Managers, Social Workers, Behavioral Health Specialists, Community Health Workers, Medical Assistants, Serious Illness Care Navigators, Value-Based Care Navigators, Care Management Coordinators, and Physician Advisors. Our teams operate across Primary Care offices, Specialty clinics, community schools, emergency departments, and inpatient settings, serving both pediatric and adult patients.

Scope of Work: Care Management team members deliver high-quality patient care under the guidance of the Care Management Leadership team, adhering to Corewell Health policies. They recognize the importance of care partnerships with patients and families to achieve optimal outcomes. Care Managers are vital members of the interdisciplinary team, advocating for patients, ensuring their voices are heard, and connecting them to necessary services. Depending on the location, their expertise may support end-of-life care, transitions from home to other care levels, mental health support, or education on specific diagnoses, with a focus on whole-person care and social determinants of health.

Qualifications:

  • LMSW - Master Social Worker license from the State of Michigan upon hire required
  • LLMSW candidates will be considered
  • Master's Degree in Social Work required
  • Minimum of 2 years relevant experience
