The JFS Aging Care Manager works with individuals 55 years of age and older and their loved ones to problem solve and guide them on their paths to safety, security, wellness, fulfillment, and future planning. Together, the Aging Care Manager, the client and family develop and implement a personalized care plan that meets the individual’s needs to support their quality of life. The JFS Aging Care Manager will connect individuals to programs that help the older adult stay independent, connected, and engaged as they age. The JFS Aging Care Manager will also engage with Holocaust survivors and their families to ensure that they are connected to services, cultural activities, and benefits that they are entitled to.
Essential Duties and Responsibilities:
- Conduct holistic intakes and assessments encompassing psychological, social, physical, and environmental concerns for clients residing in Monroe County.
- Develop and execute a comprehensive care plan with client and family.
- Collaborate, provide referrals, and connect with other agencies.
- Maintain excellent and consistent communication with clients and their caregivers.
- Provide ongoing monitoring to client and family.
- Attend internal/external meetings to advocate for clients.
- Represent the agency and provide care management services to contracted agencies.
- Participate in activities to promote JFS Care Management services in the community including staffing resource fairs and presentations on relevant topics.
- May be required to be a preceptor for social work interns and new hires.
- Other duties as requested by supervisor.
Holocaust survivors and caregivers:
- Has knowledge of intergenerational trauma.
- Uses a person-centred trauma-informed framework.
- Provide support to Holocaust survivors, second generation survivors, caregivers, and family members.
- Provides care management to the Holocaust survivor.
- Collaborates, creates, and connects individuals to cultural events, social and educational programs and activities.
- Collaborates with the Jewish Federation and other JFS programs.
- Ensures that clients are aware of and are receiving Holocaust survivors’ benefits.
- Assists with the applications and recertification for Holocaust survivor benefits as well as other entitlement programs.

Education and Experience:
- Bachelor’s degree in social work or a related field from an accredited college or university is required.
- At least 2 years of full-time experience in care management, program coordination, and assessment, with the ability to evaluate individual and family situations, address crises, and assess safety or elder abuse/neglect concerns.
- Strong knowledge of community resources, Monroe County Social Services, Social Security, and Mental Health Systems, as well as resources and interventions available for older adults, including financial and insurance challenges they may face.
- Familiarity with intergenerational trauma and the Holocaust is preferred.
- Effective interpersonal and communication skills, with comfort conducting in-person visits in clients' homes.
- Strong organizational, time management, and autonomous work skills, while also being a collaborative team member.
- Proficiency in electronic record-keeping, Office 365, Excel, and maintaining timely documentation, forms, and statistical tracking.

Salary & Benefits:
This part-time position offers an hourly rate between £21.00 - £24.20/hour. You'll be eligible to participate in our health and dental insurance plans, with JFS contributing a prorated amount annually.
