What You'll Do:
- Conduct in-depth health assessments and evaluate patient needs.
- Provide pharmacological and non-pharmacological management for patients experiencing behavioural and psychological symptoms of dementia.
- Make appropriate cost-efficient referral decisions.
- Perform inpatient geriatric consultations which include diagnosis, treatment, and ongoing follow-up.
- Work in a multi-disciplinary inpatient team to help oversee implementation of Age-Friendly Health System and ongoing operations as well as measuring process & outcome measures.
