The Older People and Hospital Social Work Team within Conwy Adult Services require a full time, Social Worker. This is an exciting opportunity to work within the Colwyn Bay Community Resource Team which is a well-established multi-disciplinary team.

You will be working with Older People and their families/carers and also people who have recently been discharged from hospital. You will use a collaborative approach with a strong focus on outcome based planning and reablement. Support will be available through regular formal supervision and day to day informal supervision, a variety of supportive group sessions and excellent training opportunities.

In order to be eligible to apply you must hold a degree in Social Work or an equivalent social work qualification, such as a DipSW. Ideally applicants will have post qualifying experience in working with Older People, families and carers.

It is essential that you have the ability to travel throughout the County on a regular basis, often to and from remote locations at short notice. Due to the nature of the work, it will be necessary to obtain a satisfactory disclosure from the Disclosure and Barring Service (DBS).

This role offers flexible working options for a work life balance. This can include adjusting your working day and hybrid working, ie a balance of office and home working.

Welsh Language Skills: The ability to communicate in Welsh is desirable for this post. We are committed to our Welsh language and are proud of our Welsh culture.
