You will manage a caseload of participants facing complex barriers to employment, delivering tailored, person‑centred support using the Individual Placement and Support (IPS) model. The role focuses on rapid job search, engaging local employers, securing competitive paid roles, and providing in‑work support to sustain employment outcomes. It is a community‑based position covering Conwy County, requiring flexibility, initiative and strong partnership working with health professionals, key referring partners, employers, and third‑sector organisations across Conwy.
