The Older People and Hospital Social Work Team within Conwy Adult Services requires a full‑time Assessing and Reviewing Officer. This opportunity is part of the Colwyn Bay Community Resource Team, a well‑established multi‑disciplinary team that works with older people, their families/carers and recently discharged patients from hospital.
Responsibilities
- Work collaboratively with older people, families/carers and recently discharged patients, focusing on outcome‑based planning and reablement.
- Conduct assessments, review cases and develop care plans.
- Provide day‑to‑day and formal supervision support, and participate in supportive group sessions.
- Use the organisation’s training opportunities to enhance professional practice.
Qualifications
- Good working knowledge of the social care and/or health care sector.
- Evidence of further relevant study such as NVQ Level 3 or above in Social Care, BTEC Trusted Assessor in Community Equipment or a DipSW.
Travel Requirements
Essential ability to travel throughout Conwy County on a regular basis, often to remote locations at short notice.
Background Check
A satisfactory disclosure from the Disclosure and Barring Service (DBS) is required.
