Overview
The Nurse Coordinator is a key role within our live-in nursing agency, responsible for managing all aspects of client placements, nurse coordination, and clinical quality assurance. Working closely with the Registered Manager and Care Advisory Team, you will oversee the end-to-end delivery of personalised nursing care in clients’ homes, ensuring safety, continuity, and a consistently high standard of service.
The core focus of our nursing team is to provide the highest quality nursing care to those who wish to receive healthcare in the comfort of their own home. Many clients requiring nursing care at home are nearing the end of life, so we provide a sensitive, compassionate and dignified pathway, offering emotional and practical support to families. You will ensure clients are matched with an appropriate nurse, and placements are well coordinated to meet clinical and regulatory guidance and standards.

Responsibilities
- Client Onboarding & Service Coordination
- Respond to new client enquiries and promote the benefits of our live-in nursing services
- Complete in-depth Care Needs Assessments with new clients, ensuring accurate clinical documentation
- Match clients with appropriately skilled nurses based on clinical needs, personality fit, and availability
- Prepare and hand over care assessments to nurses prior to placement, ensuring smooth transitions
- Maintain regular communication with clients and families to monitor satisfaction and address concerns
- Liaise with external professionals including GPs, hospital discharge teams, hospice staff, and community nurses
- Nurse Coordination & Support
- Maintain up-to-date nurse availability records and manage bookings efficiently to ensure continuity of care
- Monitor all live-in placements, providing support and guidance to nurses as needed
- Ensure nurses are fully equipped with required documentation, PPE, and medical kits before starting placements
- Conduct regular check-ins and act as a point of contact for nurses in placement
- Recruitment & Induction
- Handle all new nurse enquiries and manage the recruitment process from application to interview
- Conduct screening, interviews, and manage all necessary pre-employment checks and references
- Ensure all new nurses have the relevant training, ensuring they are prepared for live-in placements
- Monitor nurses’ compliance with NMC standards and provide support with revalidation and annual reviews
- Compliance & Quality Assurance
- Audit all clinical documentation, including care plans, risk assessments, and nurse weekly summaries
- Ensure all placements meet regulatory requirements, internal standards, and care quality expectations
- Maintain accurate records relating to nurse training, availability, and placement history
- Support ongoing training and development of nurses by sharing clinical resources and updates
- On Call & Out of Hours Support
- Participate in the on-call rota to support nurses and clients outside of office hours
- Provide timely clinical advice and problem resolution during out-of-hours periods
- Escalate urgent issues as necessary to ensure client safety and nurse wellbeing
- Administrative & Operational Duties
- Manage all relevant documentation related to client care and nurse bookings
- Assist with updating nurse profiles and creating client-facing nurse introductions
- Contribute to marketing and promotional efforts, including reviewing website content or attending events
- Collaborate with internal teams (IT, HR, Compliance) to support continuous service improvement
Qualifications
- Registered Nurse with current NMC registration
- Experience in developing and implementing care plans
- Proficient in IT systems relevant to nursing and patient care
- Excellent organisational skills to manage multiple tasks and priorities
- Ability to work collaboratively within a healthcare team
