Constance Care Limited is looking for an Hospital to Home Care Assistant to help clients after hospital discharge. You will support individuals at home, providing personal care, medication assistance, mobility support and companionship to help them regain independence.
You will work either 7am-3pm or 3pm-10pm, with up to 30 guaranteed hours per week. Full training is provided, plus paid induction, mileage payment, and opportunities for career progression within the CCH Group.
