The advanced nurse practitioner will work as part of the primary healthcare team providing Personal Medical Services to patients registered with the Practice. The postholder will be an experienced Nurse Practitioner who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care.
Main duties of the job
- To manage a clinical caseload, dealing with presenting patients' needs in a primary care setting.
- To deliver a high standard of patient care using advanced autonomous clinical skills and a broad in-depth theoretical knowledge base.
- To make professional clinical decisions within the role of autonomous practitioner applying evidence-based practice and help lead on provision of nurse access for acute clinical conditions.
- To practice within the NMC Code of Professional Conduct.
- To provide advice and accept referrals from other healthcare professionals.
- To prescribe from an agreed formulary and refer to GPs as necessary.
- To help provide specialist education and training regarding acute/chronic clinical conditions to other nursing staff and students.
- To ensure the highest standards of care are provided for the patients.
- To contribute to the development of the nursing services offered by the practice.
- Ensure patient records are recorded accurately and in a timely manner.
- To provide clinical leadership within the practice when required.

This partnership consists of 6 partners and 4 salaried doctors responsible for approximately 17,000 patients in Consett and surrounding areas. The position of Nurse Practitioner is challenging, and our patient list size generates a heavy and demanding workload.
Job responsibilities
- Triage and treat patients wishing to see a healthcare professional making any necessary referrals to other members of the primary and secondary healthcare team(s).
- To support patients and colleagues in adopting safe infection control measures for the Practice according to local guidelines.
- To refer patients directly to other health professionals as needed and collaborate with health professionals across primary and secondary care using locally agreed guidelines.
- To prescribe from the independent/supplementary nurse prescribing formulary and refer to GPs as necessary.
- To contribute to the development of clinical protocols for the nursing team when required.
- Help develop and set up new patient services and participate in initiatives to improve existing patient services when required.
- Instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports.
- To assist in clinical audit and the setting and monitoring of standards of care and implement standards for quality assurance.
- Understand the importance of recognition, observation, documentation, communication, and referral of both adult and child safeguarding concerns and be aware of local guidance.
- May be required to assess and treat appropriate patients in their own homes.

- Significant post-registration clinical experience (3-5 years)
Experience
- Minimum 2 years primary care experience working without direct supervision
- Experience of undertaking a research project and applying the findings to practice
