The purpose of this dual role is to work as a Care Coordinator/Admin Assistant within the Primary Care Network. Supporting the NNHIP Project, Care Coordinating Team and Prescription Hub, for our five constituent practices within Connexus PCN: Birchwood Medical Practice (Brislington), Nightingale Valley Practice (Brislington), Priory Surgery (Knowle), Stockwood Medical Centre (Stockwood) and Wells Road Surgery (Totterdown).
Main duties of the job:
- To support the Neighborhood Health Implementation Programme (NNHIP) pilot work across the PCN.
- To support the work of the PCN multi-disciplinary team (MDT) to enable them to provide high quality patient care.
- To support the PCN Patient Care Projects and Services.
- To support the PCN to develop and maintain links with our patient community.
- To support the PCN Prescribing Hub.
