Responsibilities

- Safely transport clients to appointments, social activities, and community resources.
- Ensure vehicle cleanliness, maintenance checks, and comply with all road safety regulations.
- Work collaboratively with healthcare professionals to deliver agency-compliant care plans.
- Monitor and report any changes in client's health or needs, following safeguarding policies.
- Ensure all care practices adhere to relevant legislation and organisational standards.
- Maintain accurate and confidential records in line with agency compliance requirements.

Qualifications
- Previous experience as a Care Support Worker, Reablement Support Worker, or similar role is preferred.
- Valid driving license with a clean driving record.
- Experience driving clients safely, including those with mobility or other assistance needs.
- Understanding of agency compliance standards and care regulations.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Commitment to promoting equality, diversity, and inclusion in the workplace and care delivery.
- Relevant certification or willingness to undertake training in reablement or social care.
- Basic literacy and numeracy skills required.
- DBS check and other statutory clearances as applicable.
