We are seeking a compassionate and dedicated Adults Social Worker specializing in Learning Disabilities (LD) and Autism (A) to join our inclusive and diverse team. In this role, you will support adults with learning disabilities and autism to live empowered, fulfilling lives while ensuring their rights and well-being are prioritised.
Responsibilities
- Conduct thorough assessments and develop person-centered care plans in collaboration with individuals, families, and multi-disciplinary teams.
- Support adults with learning disabilities and autism, including those who may be unauthorised or at risk, to access appropriate services and safeguards.
- Advocate for the rights and needs of service users to promote independence, choice, and social inclusion.
- Monitor and review care plans regularly, adjusting support as needed in response to changing circumstances.
- Work collaboratively with healthcare providers, community organisations, and legal entities to ensure holistic care.
- Maintain accurate and confidential records in compliance with data protection regulations.
- Participate in safeguarding procedures and contribute to investigations when necessary.
- Promote equality, diversity, and inclusion in all aspects of your work.
Qualifications
- Relevant social work qualification and registration with the Health and Care Professions Council (HCPC).
- Experience working with adults with learning disabilities and/or autism, including those with unauthorised status or complex needs.
- Strong knowledge of the Mental Capacity Act, Care Act, and safeguarding legislation.
- Excellent communication, interpersonal, and advocacy skills.
- Ability to work independently and as part of a multidisciplinary team.
- Commitment to ongoing professional development and reflective practice.
- Demonstrated understanding of and commitment to equality, diversity, and inclusion principles.
Day-to-Day
- Visiting service users in their homes or community settings to assess needs and provide support.
- Collaborating with colleagues and external agencies to coordinate services effectively.
- Attending multidisciplinary meetings and case reviews.
- Responding promptly to safeguarding concerns and emergencies.
- Documenting assessments, plans, and outcomes with clarity and accuracy.
- Engaging in training sessions and team supervision to enhance skills and knowledge.
- Promoting an inclusive environment that respects and values diverse backgrounds and experiences.
