We are seeking a compassionate, dedicated Adults Social Worker to join our dynamic Transitions Team. This role is pivotal in supporting young adults as they transition into adult social care services, ensuring they receive personalised, respectful, and inclusive support. As part of our team, you will work collaboratively with individuals, families, and multidisciplinary partners to facilitate smooth and empowering transitions that promote independence and well-being. Responsibilities- Conduct comprehensive assessments of young adults’ social care needs during their transition from child to adult services.
- Develop and implement personalised care plans that respect individuals’ choices, cultural backgrounds, and aspirations.
- Collaborate with healthcare providers, education professionals, housing agencies, and community organisations to coordinate holistic support.
- Advocate for clients’ rights and needs, ensuring services are accessible and inclusive.
- Monitor and review care plans regularly to adapt to changing circumstances.
- Maintain accurate and confidential case records in accordance with local and national policies.
- Participate in multidisciplinary team meetings and contribute to service development initiatives.
- Support clients and families through periods of change, promoting empowerment and autonomy.
- Professional qualification in Social Work (e.g., Diploma in Social Work, BA/MSW) and registration with the relevant Social Work regulatory body.
- Experience working with young adults and understanding of transition processes within social care.
- Knowledge of relevant legislation, frameworks, and best practices in adult social care and safeguarding.
- Strong communication, interpersonal, and advocacy skills with a commitment to equity and inclusion.
- Ability to work independently and as part of a diverse team, demonstrating cultural sensitivity and emotional resilience.
- Commitment to continuous professional development and reflective practice.
- Visiting clients in their homes or community settings to understand their needs and preferences.
- Liaising with schools, healthcare teams, and family members to plan coordinated support.
- Attending and contributing to case conferences and review meetings.
- Managing documentation, including risk assessments and service evaluations.
- Responding promptly to crises or safeguarding concerns with appropriate interventions.
- Engaging in training and supervision sessions to enhance skills and knowledge.
