Job Summary: We are seeking a compassionate and dedicated Access to Work Support Worker to join our team. The ideal candidate will work with individuals who have disabilities or long-term health conditions, helping them access and thrive in their workplace or educational environment. Key Responsibilities: Support Provision: Provide practical support to individuals in the workplace or education settings, in line with Access to Work guidelines. Ensure the individual\’s needs are met, supporting their independence, productivity, and well-being. Assessment and Liaison: Conduct assessments of individual needs in collaboration with the person requiring support. Review and adjust support plans as required, to ensure they meet the individual\’s evolving needs. Documentation and Reporting: Maintain accurate records of support provided, progress, and outcomes. Confidentiality and Data Protection: Adhere to all confidentiality and data protection policies when managing sensitive information regarding individuals and their circumstances. Training and Guidance: Assist individuals in learning how to use assistive technologies or any other tools necessary to support their role. Ongoing Support: Monitor and assess the effectiveness of the support plan on an ongoing basis, adjusting as necessary. Provide emotional and practical support to help the individual navigate any challenges that arise in their role. Person Specification: Previous experience of working with individuals with disabilities or long-term health conditions. Good communication skills, with the ability to interact with a diverse range of individuals. Ability to assess needs and tailor support to the individual. Strong organisational skills with the ability to manage multiple tasks effectively. Confidentiality and sensitivity when working with personal and confidential information. Empathy and patience, with a commitment to supporting individuals to reach their potential. Experience in assistive technology or knowledge of workplace adjustments. Knowledge of relevant health and safety regulations and their application in workplace settings. Working Conditions: Flexible working hours may be available depending on the needs of the individuals supported. Occasional travel between work sites.
