As part of the transformation work being completed in Adult Social Care, linked to the diagnostic work carried out last Summer, I am looking for 2 Social Workers to join the Mental Health Transformation team. These are small, dedicated teams of people who are interested in innovative practice and work across the County on several exciting initiatives. The role includes completing case work, ranging from the application of our strength-based approach through to taking funding requests to consistency practice meetings.

Key Responsibilities:

  • In-depth knowledge of the Care Act 2014, and other relevant legislation.
  • Understanding of person-centred approaches and how to implement them in practice.
  • Knowledge of safeguarding principles and procedures.

Essential Criteria:

  • Qualification: Recognised Social Work qualification (e.g., BA or MA in Social Work).
  • SWE Registration
  • Skills: Excellent communication, assessment, and case management skills.
  • Personal qualities: Compassionate, resilient, and committed to working with vulnerable individuals.
  • Ability to work independently and as part of a multidisciplinary team.
  • Car Driver: A full UK driving license and access to a vehicle is required for this role.
