New opportunity for a 16+ team based in North West England!

Responsibilities

  • Manage a caseload of young people aged 16-18 within the Children in Care pathway.
  • Undertake statutory visits, reviews, and care planning in line with legislation and local procedures.
  • Work collaboratively with Personal Advisors, IROs, education providers, housing, health services, and other professionals to support the transition to independence.
  • Ensure young people are prepared for leaving care by promoting their participation in pathway planning and decision-making.
  • Carry out comprehensive assessments and prepare court reports as necessary.
  • Ensure that safeguarding measures are in place and regularly reviewed.
  • Maintain accurate and up-to-date case records and reports on the local authority\\\'s case management system.

Requirements

  • Qualified Social Worker with a Social Work England registration.
  • Minimum of 2 years post-qualified experience within children\\\'s social care, preferably in a CIC or Leaving Care Team.
  • 3 years perm experience.
  • Strong understanding of the Children Act 1989, Care Planning Regulations, Leaving Care Guidance (2010), and related legislation.
  • Experience working specifically with older adolescents (16-18) and supporting their transition into adulthood.
  • Enhanced DBS on the Update Service or willingness to apply for one.
