Responsibilities

  • Ensure placements meet assessed needs, quality standards, and remain cost-effective.
  • Conduct reviews across commissioned services, focusing on outcomes, safeguarding, service user voice, and value for money.
  • Lead strengths‑based reviews in line with Care Act principles under Senior Practitioner guidance.
  • Update and implement outcome‑focused support plans, promoting independence and effective delivery.
  • Monitor commissioned services against contractual standards, quality, and outcomes.
  • Support commissioning delivery plans and contribute to ongoing market development within key service areas.
