About the Role
- As part of the Hospital Discharge Team, you will:
- Manage a caseload of patients requiring safe and timely discharge from hospital settings
- Assess individual needs and develop, review, and implement effective discharge plans
- Complete relevant assessments, documentation, and reports to support decision‑making
- Work closely with healthcare professionals, families, and multi‑agency partners to coordinate care
- Ensure individuals experience a smooth transition from hospital to home or appropriate care settings, promoting their safety, independence, and wellbeing
Requirements
- Qualified Social Worker (SWE registered)
- Previous Hospital experience
- Strong safeguarding knowledge
- Ability to work effectively in a fast‑paced environment
- Eligible to work in the UK
- Drive and have access to a car
- Post‑qualified experience
- Degree level or equivalent in Social Work
