Job Description:

  • Ensure placements meet assessed needs, quality standards, and remain cost-effective
  • Conduct reviews across commissioned services, focusing on outcomes, safeguarding, service user voice, and value for money
  • Lead strengths-based reviews in line with Care Act principles under Senior Practitioner guidance
  • Update and implement outcome-focused support plans, promoting independence and effective delivery
  • Monitor commissioned services against contractual standards, quality, and outcomes
  • Support commissioning delivery plans and contribute to ongoing market development within key service areas
