Connect2SocialWork are urgently looking for experienced Social Workers and Senior Practitioners to join their Adults Services.

Role Responsibilities:
- Demonstrated experience in undertaking Section 9 Care Act 2014 assessments, identifying eligible needs, and determining appropriate care and support provisions.
- Skilled in completing unplanned support plan reviews, ensuring responsive reassessment and timely adjustments to care packages in line with changes in need or risk.
- Proficient in risk assessment and safeguarding, applying legislative frameworks effectively across both mental health and physical disability cases.
- Practical experience supporting adults with mental health needs, with working knowledge of the Care Programme Approach (CPA) and the interface between the Mental Health Act and the Care Act.
- Strong understanding of physical disability services, including long-term conditions, mobility and access needs, rehabilitation, and technology-enabled care solutions.
- Ability to manage complex, dual-diagnosis cases involving coexisting mental and physical health conditions, coordinating across agencies and specialisms.
Requirements:
- Registered with Social Work England (SWE)
- Eligible to work in the UK
- Full UK driving licence and access to a vehicle
- Post-qualified experience in social work
- Degree or equivalent qualification in Social Work
