Halton Council are seeking a skilled and experienced Social Worker (Level 3) to join our HICaF Discharge Team . This critical role will support the safe and timely facilitation of hospital discharges, particularly as we approach the busy winter period and address the growing demands faced by the Acute Trust.

Key Responsibilities:

  • Work collaboratively to ensure safe and efficient hospital discharges.
  • Manage a complex caseload, working independently and efficiently.
  • Provide support and intervention for individuals and families during transition from hospital to community-based care.
  • Liaise with multidisciplinary teams to ensure seamless and person-centered care.

About You:

  • Previous experience working in an Integrated Hospital Discharge Team.
  • Knowledge of Eclipse (desirable).
  • The ability to hit the ground running in a fast-paced environment.
  • A full driving license and access to a vehicle (essential).

Qualifications

  • A degree in Social Work or a related field is essential.
  • Strong interpersonal skills with the ability to build rapport with diverse populations.
  • Excellent communication skills, both verbal and written, for effective documentation and reporting.
  • Ability to work independently as well as part of a multidisciplinary team.
