We are currently recruiting for an experienced Social Worker to join the Complex Care Team in Runcorn to support with a service backlog and ongoing case management. This is an excellent opportunity for a qualified and confident practitioner who has successfully completed their ASYE programme and has experience working within adult social care and complex case management.

Key Responsibilities

  • Completing Care Act Assessments
  • Undertaking outcome-focused reviews
  • Carrying out Mental Capacity Assessments and Best Interest Decisions
  • Supporting with safeguarding enquiries under Section 42
  • Completing CHC checklists and attending DST meetings
  • Participating in duty rota responsibilities
  • Working collaboratively within a multi-disciplinary team (MDT) approach alongside internal services and external partner agencies

Requirements

  • Qualified Social Worker with Social Work England registration
  • Completion of ASYE programme
  • Previous experience within Adult Social Care / Complex Care
  • Strong knowledge of the Care Act, Mental Capacity Act, and safeguarding legislation
  • Ability to work independently and manage a varied caseload
