The Intermediate and Frailty Team in Runcorn supports adults who are experiencing reduced mobility, cognitive decline, or increasing care needs. As a Social Work Assistant, you will play a key role in helping individuals maintain independence and access the right support at the right time. You will work alongside Social Workers, health professionals, and community services to deliver high-quality, person-centred support.
Key Responsibilities
- Support Social Workers by gathering information, completing initial visits, and assisting with strengths-based assessments.
- Help develop and review care and support plans to promote independence and wellbeing.
- Liaise with families, carers, and partner agencies to coordinate support and share relevant information.
- Assist with arranging services such as home care, reablement, equipment, and community resources.
- Contribute to safeguarding enquiries by reporting concerns promptly and accurately.
- Support timely hospital discharges by coordinating practical arrangements and ensuring care packages are in place.
- Maintain clear and accurate case notes in line with council policies and procedures.
Skills & Experience Required
- Previous experience in a social care, community support, or health-related role.
- Understanding of adult safeguarding, Mental Capacity Act, the Care Act 2014.
- Strong communication and organisational skills.
- Ability to work independently and as part of a multidisciplinary team.
- Confident in building relationships with adults, families, and partner agencies.
