We have an exciting role at Halton Council, to support our hospital discharge team. You will need to have experience of hospital social work, including the Home First approach is desirable. Duties include strengths based assessment and support planning, mental capacity act assessments/best interest decision making. You will be part of the duty rota. The role is largely onsite due to the nature of the work. You will be joining a friendly team, with good manager presence and support.
Key Responsibilities:
- Work collaboratively to ensure safe and efficient hospital discharges.
- Manage a complex caseload, working independently and efficiently.
- Provide support and intervention for individuals and families during transition from hospital to community-based care.
- Liaise with multidisciplinary teams to ensure seamless and person-centered care.
About You:
- Previous experience working in an Integrated Hospital Discharge Team.
- The ability to hit the ground running in a fast-paced environment.
- A full driving license and access to a vehicle (essential).
Qualifications:
- A degree in Social Work or a related field is essential.
- Strong interpersonal skills with the ability to build rapport with diverse populations.
- Excellent communication skills, both verbal and written, for effective documentation and reporting.
- Ability to work independently as well as part of a multidisciplinary team.
- Demonstrated skills in mentoring and supervising others are advantageous.
- A commitment to continuous professional development and staying updated on best practices in social work.
