Halton's Intermediate and Frailty Team provides timely, person-centred support to adults with complex, changing, or deteriorating health needs. We work to prevent hospital admission, support early discharge, and help individuals maximise their independence at home. We are now seeking a skilled and motivated Band 6 Occupational Therapist to join our friendly and highly collaborative team.

Responsibilities

  • Complete holistic assessments and deliver tailored interventions to adults with frailty, functional decline, or short-term rehabilitation needs.
  • Work closely with physiotherapists, nurses, social workers, and wider MDT colleagues to deliver safe, effective care.
  • Support safe discharge planning and admission avoidance pathways.
  • Provide clinical reasoning, risk assessments, and clear, person-centred care plans.
  • Contribute to service development and continuous improvement initiatives.

Qualifications

  • Is a qualified Occupational Therapist with HCPC registration.
  • Has post-qualification experience within intermediate care, frailty, rehab, community therapy, or a similar setting.
  • Is confident working independently and managing a varied caseload.
  • Is passionate about promoting independence and improving quality of life for service users.
