Halton Council are looking for a Physiotherapist to work in Halton's Intermediate Care and Frailty Service (HICaFS). The service includes Urgent Community Response (UCR), Intermediate Care Beds, and Reablement, working to prevent hospital admission, support early discharge, and help individuals maximise their independence at home. We are now seeking a skilled and motivated Band 6 Physiotherapist to join our friendly and highly collaborative team.
About the Role:
- Complete comprehensive physical assessments and deliver tailored rehabilitation programmes to adults with frailty, functional decline, or short-term rehabilitation needs.
- Assess mobility, balance, strength, and functional ability to support recovery and independence.
- Work closely with occupational therapists, nurses, social workers, and wider MDT colleagues to deliver safe, effective care.
- Support admission avoidance and safe discharge planning through timely intervention and rehabilitation.
- Provide clear clinical reasoning, risk assessments, and evidence-based treatment plans.
- Contribute to service development and continuous improvement initiatives.
We are looking for someone who:
- Is a qualified Physiotherapist with HCPC registration.
- Has post-qualification experience within intermediate care, frailty, rehabilitation, community physiotherapy, or a similar setting.
- Is confident working independently and managing a varied caseload.
