We are currently recruiting for an experienced Social Worker to join the Complex Care Team in Runcorn to support with a service backlog and ongoing case management. This is an excellent opportunity for a qualified and confident practitioner who has successfully completed their ASYE programme and has experience working within adult social care and complex case management.
Key Responsibilities
- Completing Care Act Assessments
- Undertaking outcome-focused reviews
- Carrying out Mental Capacity Assessments and Best Interest Decisions
- Supporting with safeguarding enquiries under Section 42
- Completing CHC checklists and attending DST meetings
- Participating in duty rota responsibilities
- Working collaboratively within a multi-disciplinary team (MDT) approach alongside internal services and external partner agencies
Requirements
- Qualified Social Worker with Social Work England registration
- Completion of ASYE programme
- Previous experience within Adult Social Care / Complex Care
- Strong knowledge of the Care Act, Mental Capacity Act, and safeguarding legislation
- Ability to work independently and manage a varied caseload
