Halton Council is looking for an experienced Social Worker to join the Halton Intermediate Care and Frailty (HICAF) Team based in Widnes. In this rewarding role, you'll support adults with a range of complex needs, carrying out assessments, managing risk, coordinating care, and working closely with multidisciplinary teams to improve outcomes and promote independence.
Responsibilities:
- Conduct in-depth assessments and manage complex cases.
- Coordinate care and access to services that promote independence.
- Work closely with healthcare professionals and partner agencies.
- Complete high-quality assessments, reports, and case records.
- Manage risk and develop effective support plans.
- Participate in multidisciplinary meetings, supervision, and ongoing training.
Requirements:
- BA in Social Work (or equivalent) and registration with Social Work England.
- Experience in Adult Social Care, particularly supporting older adults, adults with learning disabilities, sensory disabilities, or rehabilitation services.
- Strong knowledge of the Care Act, Mental Capacity Act, and safeguarding legislation.
- Proficient in person-centred support planning, outcome-focused assessments, and strengths-based practice.
