Connect2Halton is currently recruiting for an experienced Qualified Social Worker to join the Short Break Service for Children with Disabilities Team within Halton Council. We are looking for a Social Worker who shares our commitment to provide services that enable families of children with disabilities to have an improved quality of life within their communities. You will have a key role in assessing the needs of disabled children who have complex needs, producing a robust, person centred, Child in Need plan and supporting them to access short break services.
Key Responsibilities:
- Undertake complex assessments for children and young people with disabilities, identifying needs and risks, and determining appropriate short break interventions.
- Lead and manage complex care planning for disabled children and their families, ensuring packages are tailored to individual needs and promote safety, inclusion, and positive outcomes.
- Coordinate and oversee joint-funded care packages, working collaboratively with health, education, and other partners to ensure integrated support.
- Provide professional leadership within the team, supporting colleagues through mentoring, reflective supervision, and promoting evidence-informed practice.
- Work collaboratively with providers, health professionals, schools, and voluntary agencies to ensure a coordinated approach to short breaks.
- Chair meetings and provide expert advice in multi-agency forums, ensuring safeguarding and best practice standards are upheld.
- Support the learning and development of staff and students, creating a positive and reflective learning environment.
Qualifications & Experience:
- Social Work England registration.
- 3 years permanent experience.
- Previous experience of working with children with disabilities.
- Ability to work under pressure and manage caseloads.
