Hackney Council is currently recruiting for an experienced Adults Team Manager to join the Integrated Hospital Discharge Service. This is a key leadership role within a fast-paced, multi-disciplinary environment, focused on ensuring safe, timely, and effective hospital discharges for Hackney residents.

About the Role

As Team Manager, you will provide strong operational leadership across the service, managing a team responsible for complex discharge planning. You will play a critical role in reducing Delayed Transfers of Care (DTOC) while ensuring residents and their families receive high-quality, person-centred support throughout their discharge journey. You will work collaboratively with health and social care partners, driving effective multidisciplinary working to achieve positive outcomes.

Key Responsibilities

  • Lead, manage, and support a team of social workers and practitioners
  • Oversee complex discharge planning and decision-making
  • Drive performance to reduce delayed transfers of care (DTOC)
  • Ensure timely completion of statutory assessments and interventions
  • Promote high-quality, outcome-focused practice across the service
  • Work closely with multidisciplinary teams to coordinate effective discharge pathways
  • Support residents and families throughout the hospital discharge process

About You

We are looking for a confident and experienced leader who:

  • Has extensive experience in hospital discharge and complex case management
  • Demonstrates strong operational leadership and team management skills
  • Has a sound understanding of statutory frameworks, including: Mental Capacity Act, Care Act, Adult Safeguarding
  • Possesses excellent communication skills and the ability to engage effectively with multidisciplinary teams
  • Thrives in a fast-paced environment and can prioritise competing demands
  • Is committed to delivering high-quality, person-centred outcomes
