Hackney Council is seeking an experienced Social Worker to join the Adult Access & Duty Team on a locum basis. This role sits within the front door of Adult Social Care, focusing on timely responses to referrals, initial assessments, and ensuring residents receive appropriate support at the earliest opportunity.

You will work in a fast-paced environment, undertaking strengths-based assessments and collaborating with partner agencies to promote independence and wellbeing.

Key Responsibilities
  • Undertake strengths-based Care Act assessments
  • Provide information, advice and signposting to residents and carers
  • Respond to referrals and manage demand within an Access/Duty setting
  • Develop person-centred support plans focused on outcomes
  • Undertake and contribute to safeguarding enquiries
  • Assess and manage risk, including applying Mental Capacity Act and DoLS principles
  • Work collaboratively with multi-disciplinary teams and external partners
  • Maintain accurate and timely case records
Requirements
  • Qualified Social Worker (Degree/DipSW/CQSW)
  • Social Work England registration
  • Experience within Adult Social Care, ideally in Access, Duty or Front Door services
  • Strong understanding of the Care Act 2014, Mental Capacity Act and safeguarding frameworks
  • Ability to manage a busy caseload and work under pressure
  • Experience working with diverse communities and complex needs is desirable
  • Knowledge of strengths-based practice and person-centred approaches is essential
