Connect2Hackney are seeking an experienced Occupational Therapist to join a dedicated Double Handed Care Project within Adult Social Care. This exciting project focuses on reviewing existing double-handed care packages, promoting independence, reducing reliance on ongoing support where appropriate, and ensuring residents receive person-centred, strengths-based interventions.

Working as part of a multidisciplinary team, you will assess, review and redesign care solutions, helping residents achieve greater independence through specialist occupational therapy interventions, moving and handling expertise, equipment provision, adaptations and assistive technology.

Key Responsibilities
  • Undertake comprehensive Occupational Therapy assessments for adults with complex physical disabilities and care needs.
  • Review existing double-handed care packages and identify opportunities to increase independence and improve outcomes.
  • Carry out functional assessments and recommend appropriate interventions, equipment, adaptations and moving & handling solutions.
  • Assess and prescribe specialist equipment, Telecare and assistive technology.
  • Work collaboratively with social workers, care providers, health professionals and carers to develop effective support plans.
  • Promote preventative approaches that reduce long-term care needs and maximise independence.
  • Monitor the effectiveness of interventions and adjust recommendations where required.
  • Maintain accurate case records, reports and assessments in line with professional standards.
  • Support safeguarding enquiries and risk assessments where appropriate.
  • Contribute to Hackney's integrated neighbourhood model, working alongside health, social care and voluntary sector partners.
Requirements
  • HCPC Registered Occupational Therapist
  • Degree in Occupational Therapy
  • Strong experience within Adult Social Care
  • Significant experience of Moving and Handling assessments and interventions
  • Knowledge of the Care Act 2014, Mental Capacity Act and relevant housing legislation
  • Experience reviewing care packages and promoting independence through strengths-based practice
  • Excellent assessment, report writing and communication skills
